You can comfortably modify your facial features in your own Megaton house or Tenpenny Suite! Just tell your robot butler (Godfrey/Wadsworth) you want a face change, and you can adjust your appearance for free.
Previously, "plastic surgery" could only be performed by Pinkerton in Rivet City after completing the "Replicated Man" quest.
